What roles does my case manager assume?
The role functions of the nursing case manager include care coordination, facilitation, education, advocacy, resource management, focusing on positive outcomes, and discharge planning. Nursing case management is interdisciplinary. Your nursing case manager’s role is the focal point of the interdisciplinary approach to your care to eliminate the feelings of isolation, territoriality, fragmentation, redundancy, duplication, lack of communication, and unexplained, unnecessary tests / procedures. Through your nursing case manager your care and recovery as a patient/client will be facilitated by collaboration, integration, coordination, continuity, consistency, interaction, and open communication. You as a health care customer and consumer can then be an informed and happier, more satisfied client of the care received. The nurse case manager strives to pro-actively maximize these efforts for you when you are in his/her care.
