What is a nursing case manager anyway?
A nursing case manager is a registered nurse who specializes in nursing case management to help facilitate recovery to the greatest extent possible to promote your recovery to the previous state of well-being prior to injury or illness, as near pre-illness / pre-injury status as possible to the optimum level of improvement obtainable. The Commission for Case Manager Certification defines nursing case management as "a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual’s health needs, using communication and available resources to promote quality, cost-effective outcomes."
